We are recruiting for a Community Matron to join our team.
This post is part of the Hampshire and Isle of Wight NHS Trust's priority for developing services for the management of people with complex long-term conditions, providing alternatives to admission and facilitating timely discharge. The post holder will proactively support high risk patients with complex or escalating health needs on the community caseloads in a step-up, step-down approach, working collaboratively with all other members of the integrated locality team and wider multidisciplinary team to establish personalised care, promoting self-management wherever possible and safe to do so.
To collaborate with healthcare professionals and wider services to identify high-risk patients with long-term conditions and complex health needs requiring intervention.
Provide evidence-based care with sound clinical decisions, recognizing the unique needs of patients with complex long-term conditions, and ensure regular review of their care plans.
Attend meetings in relation to individual patients, promoting individualised care and the involvement of other professionals in decision making.
Promote home-based community care to prevent unnecessary hospital admissions and shorten inpatient stays.
Support patients in managing their long-term conditions by increasing awareness of potential decline and encouraging proactive actions to maintain well-being and independence.
Proactively utilise remote monitoring, where appropriate, to support self-management.
Promote personalised care based around the needs, choices and wishes of individuals, facilitating discussion with the family and significant others.
Proactively manage high risk patients with escalating health needs on community caseloads in a step-up, step-down approach.
Work collaboratively across traditional role and geographical boundaries to support people.
Use knowledge, skills and time most effectively to optimise health outcomes and improve people's quality of life.
Work collaboratively with others and support the Hospital at Home/ Frailty Virtual Ward, Urgent Care Response (UCR) and Community Nursing functions within integrated locality teams.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

Registered nurse with current NMC registration
District Nurse Specialist Practice/Public Health Community OR Advance Clinical Practitioner OR a combination of Independent Prescribing (V300), Physical Assessment and History taking and at least one module that demonstrates learning in long term conditions management (Diabetes/ COPD etc.)
Independent Prescribing qualification
MSc qualification
Demonstrable knowledge of local health and social care systems
Ability to facilitate networks of practitioners to learn from each other
Able to demonstrate knowledge of current issues in the NHS and how these relate to their area of practice.
Plan, deliver and review interventions to enable people to learn and develop
Experience of interagency working

Physically capable of performing the roles and responsibilities of the post holder
Car driver with full UK driving licence
Computer literacy in Word/Excel
Demonstrate excellent communication and interpersonal skills and an ability to relate positively with a wide range of professional and non-professional staff.
