We are excited to announce new opportunities within our Hampshire and Isle of Wight All Age Continuing Care service. We are looking for people who are passionate about patient care and improving life opportunities who share our behaviours of being person centred, honest, courageous, innovative, and collaborative.
As a Senior Clinical Assessor/Practitioner (Registered Nurse, SWE or HCPC registration) in Continuing Healthcare, you will be responsible for conducting comprehensive individual assessments of patients to assess their eligibility for NHS Continuing Healthcare (CHC) to enable timely provision of care, case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with complex health needs live their best lives.
Whilst experience in Continuing Healthcare would be helpful, if you have the right skills and attitude, we can support you to develop the specific skills required for Continuing Healthcare. We offer team training opportunities and Team Leaders who are non-case holding in order to mentor and develop our Clinical Assessors with complex cases, including safeguarding, Mental Capacity Act and Court of Protection. You will be part of a wider All Age team who provide a wide range of expertise and support.

Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities. Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

The All-Age Continuing Care Team supports over 7,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, Children's Continuing Care and other joint funding and oversight of a budget of 392M. Our roles are community based with flexible working from a local office and home and is a key role to support people to live well with complex health and primary health needs. Post holders will be employed by Hampshire and Isle of Wight Healthcare NHS Foundation Trust, working in partnership with NHS Hampshire and Isle of Wight Integrated Care Board.

Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE
Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care
Experience of assessment of health and social care needs -- care planning and coordination.
Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles)
Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team)
Experience of working effectively under stressful conditions in a rapidly changing environment.
Demonstrable experience of care/case management.
Knowledge of and experience of NHS Funded Continuing Health Care criteria -- guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act)
Experience of developing care packages.
Experience of implementing and managing Personal Health Budgets
